Not New player but new to upgrading

What are the steps to upgrading. I bought materials and an upgrade tool. Placed lv 35 staff in Tool, placed a crystal in the tool. and whats next? And before anyone says it I have searched for about 2 hours and got nothing. Good high level upgrade infor but not the basic steps..

Well need 4 things
Upgrade tool
Item your upgrading
Talic your placing on the item
Gems to increase chance of the upgrade
Base chance of upgrade starts off at like 30%
The more upgrades an item has the harder it is to upgrade. So slot 1 is easier than slot 5.
Besides that it's pretty much all luck and upgrading can easily have you begging for money or a weapon to use till you get a new one becuase you just blew yours up.

Almost forgot the safty zones. Number of talics an item can have till bad stuff happens during upgrades.
Normal - 4 upgrades
Purple - 3 upgrades
Intense - 2 upgrades
St Intense - none

Also, basically here's how it goes:
Minus strong/intense weapons, you can get away with using a t1 and t2 sets (all the gems are either tier 1 or tier 2) up to the safety zone (recommend using a t1 set for the 1st slot and then a t2 set for the second slot). Pass the safety zone, highly recommend ONLY using t3 sets. As for what can happen if you fail past the safety zone, either one of these will happen:
1. Nothing happens to the item
2. You lose up to all your talics in the item, but the item itself is fine
3. Item goes poof along with all the talics.
Finally, recommend upgrading starting at either lvl 30 or 35
